Stages of caries treatment in children:
- preliminary psychophysiological preparation, distraction of the child from the treatment procedure;
- high-quality anesthesia (injection anesthesia, preliminary local application anesthesia);
- preparation of caries (removal of affected tooth tissues with maximum preservation of healthy tissues);
filling using glass ionomer cements, light and chemical curing composites (filling material is selected individually, taking into account the age of the child, bite characteristics, allergic history and the degree of resistance of tooth enamel to caries).
Permanent teeth are laid, in another unborn baby, for 17 weeks of intrauterine development, and erupt, as a rule, from the age of 6. In other words, the process of developing permanent teeth begins long before the birth of a child and continues for about 4-6 years after eruption. Of course, during this time, the child's teeth have time to "withstand": toxicosis and iron deficiency anemia of the mother, sometimes a difficult birth, then a number of colds, a large number of different medications, lack of basic oral hygiene, an abundance of delicious confectionery, carbonated drinks and much more… And so, by the time of the full appearance of a permanent tooth in the oral cavity, most often it is already affected by caries and needs treatment.
Current trends provide for an individualized and comprehensive approach to the treatment of dental caries in children. The scope of treatment is determined by age, the hygienic condition of the oral cavity, the intensity of the lesion and the clinical course of dental caries.
During the first visit, the dentist shows how to brush your teeth, recommends hygiene products, the shape and size of a toothbrush, directs the child to a dental hygienist for professional oral care and even prescribes therapeutic nutrition. All this is aimed at reducing the number of repeated patient visits and strengthening the teeth that are just erupting…
It is better to start any treatment with preliminary psychophysiological preparation. Children of primary school age can be told a fairy tale, thereby the text is synchronized with the action of the doctor and provides a good psychotherapeutic effect. Children of middle and high school age can broadcast musical works, cartoons and feature films. In such cases, it is better for the child to start listening in the waiting room, and after entering the office and sitting in a chair, he continued to listen through headphones and see on the monitor. This will maximize the distraction of the child from the treatment process.
The next stage is high-quality anesthesia, which includes not only injection anesthesia, but also preliminary local application anesthesia. Application anesthesia blocks the sensitivity of the terminal nerve endings, so the introduction of a needle, when injected with an anesthetic, is almost painless.
The stage of caries preparation is the removal of necrotized "damaged" tooth tissues. The modern approach to this stage of treatment is to maximize the preservation of hard tissues, i.e. the tooth is prepared very gently, under the control of a caries marker.
When the tooth is prepared, they proceed to the final stage – filling. Various filling materials are used to restore the tooth crown: chemical and light curing composites, glass ionomer cements. The filling material is selected for the patient strictly individually, taking into account age, allergic history, malocclusion and the degree of resistance of enamel to caries.
Sealing of fissures
Sealing of fissures – this is a modern method of primary prevention of caries. It is carried out on both temporary and permanent teeth. For this method of prevention, sealants are used – a special liquid-flowing filling material that fills natural recesses (fissures) and pits on the teeth. Many sealants contain fluoride, which contributes to the long-term fluoridation of the areas of the tooth on which they are located.
Fissures and pits - these are grooves located between the mounds of chewing teeth and depressions on the buccal and palatine surfaces of various groups of teeth. They can be open and closed; wide and short, narrow and deep. The most favorable for the development of caries are closed, narrow and deep fissures. They create ideal conditions for the fixation and reproduction of microorganisms in the oral cavity. The toothbrush is not able to clean them, since the diameter of the bristles is much larger than the diameter of the fissure itself. This leads to insufficient cleaning of soft plaque and the formation of caries.
At what age is sealing performed?
The period of teething is characterized by a low level of mineralization, which continues throughout 1,5-4-6 years since the eruption. First molars (chewing teeth) they appear in 6-8 years, 9-11 years – canines and premolars (small chewing teeth), in 12-14 years – second molars (chewing teeth). Sealing of fissures during this period is most effective while the teeth are not yet affected by caries. In a temporary bite, preventive sealing of milk molars is carried out from 3 to 5 years.
Stages of sealing.
The technology of applying the sealant is simple, painless and absolutely safe. Sealants are similar in their properties to light-cured seals. Due to their fluidity, they more easily penetrate into narrow and deep fissures, up to their very bottom.

1. Cleaning the tooth with a brush and hygienic paste. Sometimes it is necessary to use ultrasonic and sandblasting devices (with poor hygiene). A clean and dry tooth surface is the key to the success of sealing. |

2. Isolation from saliva. 3. Applying the gel, that makes the tooth surface rough. The adhesive strength of the sealant with the tooth tissues increases. After 40-60 seconds, the gel is washed off. |

4. Apply a sealant and carefully distribute it into all the recesses and pits. |

5. The curing of the sealant is carried out within 20-40 seconds using an LED lamp. |

Durability of sealing
The safety of sealants for the first time in years is 80%, after 5 years about 70%, and by 10 years of service remains 30%. After the procedure, the risk of caries is reduced by 70-80%. Sealants can last a lifetime. Often in adult patients, you can see teeth covered with sealant even in childhood.
In case of loss of the sealant, it is impossible to talk about failure, since for some time the surface of the tooth was protected from acid-forming bacteria, which means from caries. With regular preventive examinations, the doctor will be able to monitor the condition of the sealant and correct the shortcomings in a timely manner.
