When does a dentist use a microscope?

The rationality of the operation of the microscope is fully justified at all stages of dental treatment, including during primary diagnosis. Its use is in demand in the following situations:

Caries treatment

Caries can be attributed to the most common dental diseases. The main cause of caries is the failure of a person to observe individual oral hygiene, which leads to the appearance of plaque on the tooth enamel, which is firmly attached to the surface. Plaque gradually thickens due to the ingress of mineral salts into it, as a result of which a plaque is formed, where a large number of bacteria are concentrated. Bacteria in the process of their vital activity secrete lactic acid, which destroys the enamel of the tooth.

The treatment of caries involves the removal of tooth tissues affected by caries and the strengthening of weakened tissues with the help of filling materials.

Using the cofferdam

We use cofferdam in 80% of cases of therapeutic treatment.

Cofferdam is a latex plate designed to isolate the tooth during treatment from the humid environment of the oral cavity. Cofferdam protects the mucous membrane of the patient's oral cavity from aggressive antimicrobial fluids that are used to wash the root canals of teeth. Cofferdam also warns against the risk of the patient inhaling very small endodontic instruments. When setting a light-cured seal, the vapors formed during the patient's oral breathing negatively affect its quality. The cofferdam allows you to isolate the filling material from the vapors of oral respiration. The use of cofferdam is simply irreplaceable with increased sensitivity of the mucous membrane of the palate and the posterior parts of the tongue, i.e. with an increased "vomiting" reflex.

Thus, treatment using cofferdam has a number of significant advantages.

In addition to the fact that the use of cofferdam provides the maximum effect of treatment, it allows the patient to feel more comfortable.

Let's try to briefly describe the caries treatment regimen for you.

Initially, the injection site is anesthetized, an anesthetic gel is used for this (the mucosa is lubricated), then anesthesia is performed using hypoallergenic anesthetics.

Next, the preparation of the carious cavity is carried out, with the help of hogs, the enamel and dentin layer of the tooth exposed to the carious process is removed. If you leave caries under the filling, the tooth will gradually collapse. In order to determine the dental tissue damaged by caries, a special dye (caries marker) is used, which does not stain a healthy tooth. Dissecting a tooth is a labor and, believe me, a doctor has no interest in drilling healthy tissue.

After preparation (caries cleansing) of the carious cavity, a pickling gel is applied, which creates a microcellular relief on the surface of the cavity. Then a bond (a kind of glue) is introduced into these cells to bond the seal with the tooth tissues. Bond, as a rule, is illuminated by a lamp.

Then, in small portions, 2-3 mm each, layer by layer, a composite material is introduced into the cavity, which is illuminated by a light-curing lamp, a special blue light. The composite material freezes under the influence of light of a certain wavelength. It is impossible to put a large amount of composite material into the cavity at once, it simply will not light up, the seal will be raw from the inside and will soon fall out. The material is applied in layers, in small portions of 2 mm, each portion is highlighted separately.

In order for the seal to be tightly located to the adjacent teeth, the doctor applies various matrices, wedges the teeth with special wedges. Otherwise, the patient will constantly experience discomfort when food gets between the teeth.

After setting the seal, it must be thoroughly sanded and polished using various abrasive pastes and polishing heads. If this is not done, secondary caries will develop on the border between the filling and the tooth in the near future. Plaque is glued to the rough surface, which is a breeding ground for caries-causing microbes.

It is also necessary to cover the seal with a special water-repellent resin, which will significantly increase its shelf life.

This whole process, with the correct implementation of all technologies, takes a lot of time, from an hour minimum to two and a half hours maximum.

If you were given a seal in 20-30 minutes – this should alert you! The doctor cannot perform all the manipulations necessary for the treatment of caries during this time.

And now let's try to describe the treatment of pulpitis.

The patient came to the dentist with a small hole, and the tooth almost didn't hurt, and then suddenly the nerve was removed from the tooth, and even a huge "hole" was drilled…

On top of the tooth is covered with a fairly strong fabric – enamel. With the development of caries on the enamel, a carious lesion occurs in the form of a small hole in the tooth. Under the enamel there is a less durable tissue - dentin, which undergoes the carious process much faster. The tooth seems to deteriorate faster from the inside than visually from the outside. When the doctor opens the hole in the enamel and removes all the dentin affected by caries, that's when this huge cavity is formed.

Inside the tooth there is a neurovascular bundle (the so-called "nerve" in the people).

When the carious process spreads deep into the tooth and reaches the "nerve", then it, this nerve, has to be removed. In addition to the tooth cavity, the neurovascular bundle is located in the root canals. Cleansing and filling of root canals is called endodontic treatment. This is a technologically complex process. The root canals of the tooth, most often, have a curved shape and a complex structure. A lot of tools and electronic equipment are needed for the treatment of channels. A doctor who is not armed with this arsenal will not be able to treat a tooth qualitatively.

Initially, the channels run the entire length and are expanded with various hand tools, then they give the channel the necessary shape with the help of machine tools, then the channels are thoroughly washed with antimicrobial solutions, dried and sealed tightly along the entire length. If the channels are not sealed for the entire length, sooner or later inflammation will develop in the tooth. If the channels are not thoroughly rinsed with antiseptic solutions before filling,

At the end of treatment, the doctor should show the patient a control X-ray, where it is clearly visible that all root canals are sealed along the entire length, up to the tip of the root.

Why is a tab better than a seal? The seal is an old proven method.

Why can't you close your mouth during treatment without latex

For high-quality dental treatment, modern dentists use a cofferdam (latex curtain). But in some cases, this device cannot be used in work, for example, when the tooth is severely destroyed and it is impossible to fix the retaining device (clamp), or when the cofferdam even interferes with good access to the canal. And then the doctor asks the patient not to close his mouth during treatment. For what? Most modern filling materials require a perfectly dry surface, so the slightest ingress of liquid (saliva or moisture released during breathing) significantly reduces their "sticking" ability, which affects the service life of the seal. And when treating dental canals, it is important that the oral fluid, which always has a lot of microbes, does not penetrate into the canal. That's why cofferdam is so necessary. In rare cases, if the doctor asks you to keep your mouth open for a while, be patient a little, and the treatment will be of high quality.

Why treat a tooth if sweat

Tooth extraction is an extreme measure when all methods for its preservation are no longer possible or human health is under threat. Will I choke on saliva and will I be able to breathe well if the treatment takes place lying down?

Most patients are afraid of pain, they are worried about "drilling" or injection. When the patient is sitting in a chair, he begins to feel discomfort due to loss of control. The inability to stop the procedure when he wants it, scares the patient more. The patient may also experience fear due to his own unsuccessful experience in the past. When the doctor does not demonstrate and does not tell what is happening, the patient may have the feeling that the doctor does not solve the problem or even treats something that hurts.
